This role plays a key part in our remote physiological monitoring (RPM) program by providing ongoing clinical oversight, developing patient care plans, and offering support and education to help patients improve their health outcomes. As a Clinical Care Manager you will monitor and review patient vitals and care plans, deliver patient education and support and coordinate care with physicians.
This PRN position involves providing over-the-phone assessments, documenting encounters, and providing hospice instructions. The role requires coordinating patient care with families and hospice staff, and performing direct patient care home visits as assigned. The position is remote but requires candidates to live within an hour of Cleveland.
The Care Manager provides patients with on-demand telephone-based care management services through a 24/7 Nurse Advice Line, assesses symptoms/concerns to determine the urgency and type of care needed, and refers to or schedules appointments with providers as appropriate. The Care Manager also gives health information and advice. The goal is to reduce unnecessary visits to the clinic and emergency department.
The Triage RN, Access provides clinically expert virtual triage service, functioning as a liaison between the patient and their physician. They assess the presenting problem, utilize evidence based triage protocols to identify acuity, and develop appropriate care advice and next-step recommendations. The role requires excellent customer service, communication skills, and the ability to function as part of an effective team that prioritizes patient care.
